The building originally featured five duplex apartments every two floors, showcasing an innovative design that provided some units with bedrooms above the main living areas. Over time, the building underwent significant renovations, including a conversion to rental apartments in 1917 and further modifications in the 1940s, which included stripping away much of its original ornamentation.

Today, 121 Madison Avenue houses 127 apartments, offering a range of layouts from studios to four-bedroom units.
